The UA&P Crisis Management Committee (CMC) would like to inform everyone that effective March 21, UA&P will implement the revised entry protocol below:

- Present your UA&P ID to the guard at the entrance gate (Pearl Drive or DCB) and undergo a temperature check. Those with a temperature of 37.5°C and above will not be allowed entry.
- Present proof of full vaccination (VaxCert or vaccination card) if you are entering for the first time. This is a one-time presentation to Security who will take a photo of your card. Alternatively, employees* may upload their proof of vaccination here.
- Students who intend to join limited face-to-face classes and/or participate in on-campus activities will need to submit additional documents in this campus pre-entry requirement form. This form needs to be accomplished only once, and students are encouraged to submit it by the first week of April.
Once on-campus, everyone is reminded to follow health protocols at all times.
*We still encourage employees to advise AFM through [email protected] for onsite work to ensure a smoother entry and stay in the University. In addition, if employees wish to conduct on-campus academic and non-academic activities involving students, please accomplish this on-campus activity application form. Employees are likewise encouraged to guide students in submitting their campus pre-entry form.
